Structural Ultrasound Scanning

All pregnant women are advised to have a scan for structural abnormalities by ultrasound at 20-22 weeks. Most foetal defects, around 80%, will be detected at this stage.

This is a structural survey of the skull, intracranial structures, cleft lip, chest, heart, abdomen, kidneys, bladder, genitalia and limb abnormalities. Foetal growth parameters will be recorded.

Three dimensional (3D) and four dimensional (4D) ultrasound images can be obtained at the same setting. Our centre uses a state of the art ultrasound machine to achieve the best possible images.

If foetal abnormality is detected, we shall search for other ultrasound abnormalities. The couple will then be counselled on the diagnosis, the need for further investigations like employing amniocentesis for karyotype and aCGH testing for a comprehensive genetic survey, so as to have more exact information to counsel on the outcome and the prognosis, any appropriate prenatal or postnatal treatment for the abnormality and the recurrence risk for the next pregnancy.

According to the law of Hong Kong, termination of pregnancy is allowed for a very abnormal foetus before 24 weeks of gestation.

Some foetal abnormalities may not be evident till after 24 weeks, for example, congenital heart disease, hydrocephaly and short limbs. If the foetus has structural abnormality, we may arrange you to consult the related paediatrics or surgical specialist for counselling as necessary. If the foetus has genetic abnormality, you and your family may be referred to the clinical geneticist for genetic counselling. We then will manage you jointly to achieve the best outcome.
