Female Infertility . Endometriosis

Endometriosis is a disease process in which the endometrial lining of the uterine cavity exists outside the uterine cavity. The cause is not known. The female may have lower abdominal pain, period pain or pain during sex. Some do not have symptoms. Endometriosis can affect egg quality, cause fallopian tubal blockage because of adhesions and can affect embryo implantation and development.

Diagnosis:

  • Laparoscopy is required for diagnosis in most cases.
  • For mild cases, small endometriotic deposits are found in the peritoneal cavity.
  • For severe cases, ovarian endometriotic cysts; adhesions and scarring of the peritoneal cavity; adenomyosis( endometriotic deposits in the uterine wall) may be present.

Treatment:

  • Medicine - medicine used for treating endometriosis will not improve infertility.

  • Surgery - includes laparoscopic division of adhesions, conservative ovarian cystectomy, excision of endometrioma and extensive ablative surgery for severe endometriosis. Surgery will improve pregnancy rate.

  • Assisted reproductive technologies: IUI or IVF-ET
    - IUI can be tried for mild cases with normal fallopian tubes.
    - Surgery +/- IVF-ET are required for moderate to severe cases.
