Clinical decisions about IUI and IVF

  • IUI treatment requires ovarian stimulation to achieve modest result, but the high multiple pregnancy rates need to be considered
  • Mildly stimulated (1-2 follicles) cycles might reduce the cost and multiple birth rates but may require more cycles of treatment.
  • Unstimulated IUI: does not significantly increase pregnancy rates
  • Clomid/IUI: 5-7% pregnancy rate per cycle;
  • IVF: 7-fold higher likelihood of pregnancy;
  • ICSI: is better than IVF only in couples with severe male infertility
  • Cost-effectiveness:

    For couples with unexplained infertility and mild male factor, a primary offer of an IVF cycle was both cheaper and more cost-effective than starting with IUI or stimulated IUI followed by IVF.
